Alpha Policy

Scot Trahan Medical Massage & Rehabilitation Cancelation Policy

At Alpha Medical Massage & Rehabilitation (Scot Trahan LMT, MMP), it is my intention to provide my patients with professional Medical and Deep Therapeutic services; I will tailor each treatment to my patient’s individual needs. I do not do relaxation work, however treat your body for any pains discomforts and structural problems;  with deep techniques that may be uncomfortable and sometimes painful in the process to achieve the overall goal of being “Pain free”.

The following policies and procedures serve as a guide for my patients:

We understand that unanticipated events happen occasionally in everyone’s life including your therapist. Business meetings, project deadlines, flight delays, flooding, car problems, snowstorms or ice on roads, and illness are just a few reasons why one might consider canceling an appointment. In our desire to be effective and fair to all of our clients and out of consideration for our therapists’ time, we have adopted the following policies:

24 hour advance notice is required when cancelling an appointment. This allows the opportunity for someone else to schedule an appointment.

• If you are unable to give us 24 hours advance notice you will be charged the full amount of your appointment. This amount must be paid prior to your next scheduled appointment.

It is understood that your therapist may have to cancel due to illness, emergency or other reasons, and a 24 hr advance notice will be given when at all possible. However, this is not always possible and cancellation may happen at some other time prior to the appointment.  Alpha will not refund, but will reschedule in these cases.

No-shows

Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show”. They will be charged for their “missed” appointment and future service will be denied until payment is made, or card is updated (which can be done online from your book now log in  on our website).

Arriving late

Appointment times have been arranged specifically for you. If you arrive late your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session.

Out of respect and consideration to your therapist and other customers, please plan accordingly and be on time. 

Membership Policy

  • Your membership entitles you to (1) 60 Minute Massage Session or (2) 30 min sessions) Monthly that will be available after your payment date every month.
  • You may receive unlimited additional massages during the month for a discounted rate if on our membership.
  • Your membership and pre-paid massage session can be shared with  additional family members, if they are added to the Monthly Family Membership.
  • You may use your prepaid towards a 90 minute massage, 60 minute massage, or 3 min massage sessions. If you don’t have enough points to cover the session, you can pay the difference.
  • All Alpha Massage sessions include five minutes for Check In and Initial Consultation as well as time to undress. All sessions conclude with five minutes for Post-Consultation, guest recommendations and time to redress. That means 50 minutes of customized hands-on treatment from a Medical Massage Therapist.
  • You understand that if you are not able to come in a particular month, you still will be charged and your pre-paid massage sessions will roll-over to the next month. It is YOUR responsibility to use the credits to your membership and Alpha WILL NOT refund any charges because you didn’t use them.
  • If you are late to your appointment you will ONLY receive the remaining time.
  • If you cancel your appointment after the 24 hour window, before the appointment, you will forfeit ½ a credit .
  • If you no show on an appointment 1 credit will be used to cover the appointment.
  • Pre-paid Massages can ONLY be used by the member or listed Family Member on the policy in good standing (Meaning if your membership is suspended or frozen) no points may be used until the membership is reactivated. All memberships must be complete to term (either 6 or 12 months) before cancelation  of the membership can take place, or unless cancellation of the membership is done by Alpha.
  • If for any reason Alpha will have to be closed for a long period of time, or closing;  at least a 30 days advance notice will be given if possible, and then all memberships will either be frozen (during a long period) or canceled ( if closing).

 

Cancellation of Your Membership

If you chose to terminate your membership

  • Your membership is set up month to month and auto renews until you cancel.
  • You understand that by signing up to our membership you agree to all cancellation and other policies of Alpha.
  • When the canceling of  your membership happens for any reason, you understand that you will be charged one future scheduled Benefits Program payment, after the notification of cancelation is given by Alpha or Patient, to your debit/credit card.. You will have 30 days after this payment to use any remaining accumulated massages, after which all accumulated massages, will be forfeited from your account and your membership will be considered terminated. There will be no refund of any kind on terminated sessions, due to the patient not using the available credits within the time of membership or 30 day cancellation period.

 

If Alpha Massage chooses to terminate your membership:

For the any of following reasons Alpha Massage may cancel your membership at any time and all pre-paid sessions will be considered forfeited by you if:

  • If you have insufficient funds in your account for your monthly membership fee two consecutive months in a row from the provided Credit/Debit Card. Use of accumulated pre-paid sessions will not be allowed until a new credit card is updated in our system.
  • Inappropriate behavior from the clients will not be tolerated in any manner.
  • You have knowingly given Alpha Medical Massage false information and credit account information to process your membership properly.
  • We reserve the right to terminate or deny re-enrollment for an undetermined amount of time for not following any of our Policies & Procedures (without and refunds of payments).

 

If for any reason Alpha will have to be closed for a long period of time, or closing;  at least a 30 days advance notice will be given, and then all memberships will either be frozen (during a long period) or canceled ( if closing).
